- Why was the form below photocopied almost illegibly?
Probably because it contradicts statements by senior staff (as well as testimony later given under oath by the Residential Director and the Salvation Army H.R. Director).

The assistant to the executive Director specifically mentioned benefits because she knew they were necessary and important to me.
The single most important personnel document is not something the H.R. Director has been willing to give me. It is a paper that the D.D. Director and Residential Director asked me to sign, saying that I did not want accommodations for my "disability". Other papers signed the same day were provided.
He is legally obligated to give me all personnel file papers that have my signature, but there is no way for me to force him to obey the law.
